Student Teacher Evaluation

All student teachers in Spring Arbor's teacher education program should undergo a mid-term and a final evaluation for each placement that they have. Cooperating teachers and supervising faculty may download a copy of the form to fill out on the computer (MS Word, last update Jan. 26, 2006). To download, right-mouse on the link, select "save as ...' or "save target as ..." and pick the place on your computer where you want to save it.

The comments you wish to make can be added to the comment section or you can attach a letter of recommendation to serve as your comments.
 
This form may be signed by typing in your name and code in the proper blank. It should then be sent by email. You may also print your copy and sign it in ink, and either fax or mail it.
